Job Summary: Internal Medicine or Geriatrician to work 2-3 days/week over 65 age group population and 2-3 days/week with other adults at our Federally Qualified Health Centers. Over time this position may shift exclusively to the over 65 group. Will consult with the VNA HouseCalls program and may be called upon to occasionally visit patients in their homes. May require collaborative agreement(s) with NPs as part of our HouseCalls and/or clinic work. Job Summary: The Team Leader Nurse facilitates the implementation of the VNA Health Center (VNAHC) primary health care program through the coordination of services, policy and protocol implementation and compliance, activities and grant requirements with the support and assistance of other management and staff focused on VNAHC services. The Team Leader Nurse is responsible for supervision and training of VNA HC staff including nurses, CNAs, phlebotomists and other staff to ensure efficient clinic flow and effective nursing care. A Team Leader Nurse is available during all clinic service hours.

 


REGISTERED NURSE CARE MANAGER: AURORA – FULL-TIME HEALTH CARE HOME

Hours –M-F 8:00am – 4:30pm Occasional weekend and evening on-call responsibilities.
Qualifications: Education Nurse registered to practice in the State of Illinois, BSN preferred.


Knowledge/Skills: High degree of sensitivity to the needs of patients and families and the ability to establish constructive working relationships with them. Current driver's license in the State of Illinois and car insurance. Owns and operates a car. Maintains current CPR certification.


Type/Years Experience: Two or more years of nursing experience, Home Health experience required.


Job Summary: The VNA Registered Nurse Care Manager for Health Care Home will provide overall coordination of healthcare services for a caseload of approximately 300 patients.


REGISTERED NURSE: PART-TIME HOSPICE

Hours – Tues, Thurs, Friday: 8am to 4:30pm with on call and weekend rotations for Part-time.


Qualifications: Education Registered Nurse in the State of Illinois required. Bachelor’s degree in nursing preferred.


Knowledge/Skills: High degree of sensitivity to the rehabilitation needs of patients and their families and the ability to establish constructive working relationships with them. Current driver's license in the State of Illinois and car insurance. Owns and operates a car.


Type/Years Experience: Two or more years of generalized nursing experience, preferably with Community Health experience and/or oncology nursing experience.


Job Summary: A Hospice Staff Nurse is a registered nurse who is responsible for providing direct nursing care to Hospice and Palliative Care patients. The staff nurse works with and coordinates the care of the other team members and family members. Together the team carries out quality, comprehensive care under the supervision of the RN. The RN makes referrals to community resources as needed


REGISTERED NURSE: AURORA – PRN HOME HEALTH

Hours – PRN Varies as needed. Full-time M-F, rotate weekends, holidays and on-call.
Qualifications: Education Bachelor’s degree in nursing preferred.


Knowledge/Skills: Bilingual in Spanish a plus. Comprehensive knowledge and skill in current nursing practice; concept of multidisciplinary team approach to nursing care; knowledge of professional limitation and scope of practice within the Illinois Nursing Act.


Type/Years Experience: Registered Nurse in the State of Illinois required. Two years of nursing experience required, preferably with Community Health experience. Senior Care experience is highly preferred.


Job Summary: A Home Health Staff Nurse is a registered professional nurse who is responsible for providing direct nursing care to patients in the home. The staff nurse works with and assigns responsibility to other nursing team members and family members. Together, the team carries out quality, comprehensive nursing care under the supervision of the RN. The staff nurse also coordinates other professionals as well; PT, ST, OT, MSW. The RN makes referrals to community resources as needed.


PHYSICAL THERAPIST/PHYSICAL THERAPY ASSISTANT: AURORA & ELGIN - FULL-TIME/PART-TIME/PRN

Work Hours: M-F: 8:00am–4:30pm and as needed/scheduled.


Qualifications: Education Physical Therapist licensed in Illinois required.


Knowledge/Skills: Ability to provide physical therapy rehabilitation services and tasks involving physical activity, which includes heavy lifting and extensive bending and standing. Ability to communicate effectively, verbally and in writing.


Type/Years Experience: 1 + years Physical Therapist experience required.

 

Job Summary: Provide rehabilitation services for those clients in need of physical therapy under the direction of a physician.


CUSTODIAN/INVENTORY CLERK: AURORA – FULL-TIME HEALTH CENTER

Hours: M-F: Days


Qualifications: Education H.S. Diploma


Knowledge/Skills: Must have knowledge of methods, materials and equipment used in custodial work. Safe work practices. Ability to use a variety of custodial equipment, supplies and materials. Flooring and cleaning techniques. Prior inventory experience a plus.


Type/Years Experience: One year of experience in janitorial/ custodial work a must. Prior inventory experience a plus. Ability to lift 50 pounds or more.


Job Summary: Will clean and care for facility at Highland location during day-time hours. Will work with all VNA departments, offsite and onsite, to routinely count inventory, ensure target levels for usage are established, and orders are submitted to the Purchasing Liaison to avoid supply shortages. Rotates all inventory to avoid expired supplies. Works closely with the Directors to monitor the dispensing of supplies. Travels to offsite locations in order to perform inventory functions.


BI-LINGUAL DENTAL ASSISTANT: AURORA – PRN DENTAL OFFICE

Hours - Varies as needed.
Dental Clinic Hours: Mon. Thurs. Fri. 8-430 Lunch 45min-1hr between 12 and 2 based upon patient scheduling.

Qualifications: Education High school diploma or equivalent. Completed course work and maintain certification as a dental assistant at an accredited dental assistant program or six months related experience and/or training.


Knowledge/Skills: - Computer skills and the ability to effectively communicate with Dentist, staff and patients a must.


Type/Years Experience: Bi-lingual English/Spanish preferred. Prior chair side dental assistant experience. Experience in a fast paced environment preferred.


Job Summary: Assists the dentist with patient procedures and performs stocking, cleaning and sterilizing procedures to support dental office. To assist Dentist during examination and treatment of patients.


MEDICAL BILLER: AURORA – FULL-TIME – HEALTH CENTER

Qualifications: Education Certificate of completion from accepted training program as required by State Law. Registration on the Dept. Nurse Aide Registry.


Knowledge/Skills: Ability to perform tasks involving physical activity which includes heavy lifting and extensive bending and standing. Bilingual in English/Spanish strongly preferred. Strong computer skills required. Competencies desired include finger sticks, use of a glucometer and hemocue, urine dipstick testing, ability to use a manual blood pressure cuff and a manual scale. Phlebotomy certification required.


Type/Years Experience: Minimum 1 year CNA experience preferred.


Job Summary: A Certified Nurse Assistant (CNA) performs specific patient tasks assigned by a Registered Nurse (RN). Supportive services of the CNA assist in meeting the personal care needs, emotional comfort, and to assist the client toward independent living in a safe environment. Under the direct supervision of a Registered Nurse, performs specific assigned tasks in accordance with the plan of treatment. Performs laboratory screening procedures as a Phlebotomist.
