What is Centering Diabetes?

Centering Diabetes is a new way to receive health care. It is a group setting for your health care check ups, support, and education. Centering Diabetes is for the newly diagnosed as well as those who have been dealing with diabetes for many years.

Benefits of Centering Diabetes

  • You will meet with others diagnosed with diabetes and share stories and experiences.
  • Groups meet for approximately 2 hours, giving you more time with your provider.
  • Groups start and end on time, and there is no waiting to check in.
  • You’ll learn self-care as you take and record your own blood pressure, blood sugar, and weight at each group meeting.
  • Tracking your progress with the group’s support will help keep you motivated.

What is the cost of participating in Centering Diabetes?

The fee for a Centering Diabetes session is the same as a regular doctor visit. VNA can bill Medicare, Medicaid or your insurance. If you don’t have insurance, sliding scale fees will apply.

What is a Centering Diabetes meeting like?

  • Your group will meet 8 times per year.
  • You will have some one-on-one time with your provider.
  • Guest speakers are invited to discuss a variety of health related topics.
  • Snacks and drinks are provided.
  • Groups are available in Spanish.

Centering Diabetes is for adults only and meets at the following locations:
